مؤلفه‌های تجربۀ انگ بیماری در خانواده‌های بیماران مبتلا به اختلالات شدید روان‌پزشکی: یک مطالعۀ کیفی

چکیده

اطلاعات کمی در مورد چگونگی بروز فرایند انگ­زنی در فرهنگ­های مختلف وجود دارد. هدف پژوهش حاضر، بررسی مفهوم انگ­زنی در فرهنگ ایرانی است. به این منظور با استفاده از روش کیفی تحلیل محتوا بر روی نمونه­­­­ای متشکل از 25 نفر از خانواده­های بیماران مبتلا به اختلالات شدید روان­پزشکی به ارزیابی این مفهوم پرداخته شد. تأثیر انگ توسط دو نفر متخصص سنجیده شد.با تحلیل محتوای اطلاعات حاصله چند طبقه، که نشانگر ادراک مراقبین از انگ بود، مشخص گردید. طبقات عمده، دربرگیرندۀ این مفاهیم بودند: پنهان کاری، نگرش­های مبتنی ­بر انتقال ژنتیکی بیماری، عقاید سنّتی در جامعه نسبت به بیماران روان­پزشکی، عقاید مبتنی ­بر مشیّت الهی و پذیرش، فشار و اهمیت تفاوت­های جنسیتی در میزان انگ دریافتی. با توجه به نتایج، به ­نظر می­رسد که بیماران با اختلالات روان­پزشکی و خانواده­های آن­ها، از دامنۀ وسیعی از پیامدهای بیماری در قالب انگ رنج می­برند. پنهان­کاری و محدودیت به عنوان تجربۀ شایع در میان این خانواده­ها گزارش شدند. افزایش اطلاعات عمومی افراد در رابطه با انگ بیماری، به تعدیل نگرش­های انگ­زنی و واکنش­های منطبق­تری از سوی بیماران و خانواده­های آن­ها منجر می­گردد.

چکیده [English]

Although the stigmatization of mental illness is a long-recognized issue, yet little is known about how the problem manifests differently across cultures. The aim of this study was to account for cultural differences in describing the experience of stigma in relatives of psychiatric patients at psychiatric ward. Qualitative data were obtained based on responses to 10 open-ended questions about stigma experience in 25 participants. The impact of stigma was rated by two encoders. The content analysis of data revealed few various categories describing caregivers’ perceptions of stigma. Main categories include: concealment, limitations, genetic attribution, and traditional beliefs in society about patient, godsend and acceptance, burden, importance of gender differences in stigma of illness. Patients with psychiatric disorders and their families suffer from stigma with an astoundingly broad range of negative attributes which may interfere with various aspects of their life. Concealment and limitations are common experience in families. Publicizing stigma experience could help modifying stigmatizing attitudes in society which may lead to healthier reactions from patients, and will enhance the course of illness.
